Registration information clause

  1. The controller of your personal data is “CZĘSTOCHOWA” Cargo & Travel Agency Jan Korsak, with its registered office in Częstochowa 42-200, ul. Św. Rocha 31, operating under the brand name “PoloniaMedica”, hereinafter referred to as “PoloniaMedica”.
  2. In matters of data protection, you may contact the Controller at the e-mail address contact@poloniamedica.com, or by post: PoloniaMedica, ul. Św. Rocha 31, 42-200 Częstochowa.
  3. Your personal data obtained in connection with registration (setting up an account for the purpose of purchases - concluding a contract) will be processed for the following purposes:
    a) related to the conclusion and performance of the contract,
    b) related to complaints, pursuing any claims and compensation,
    c) for the purposes of business activity, including carrying out analyses, preparing reports and statistics,
    d) responding to letters, applications and complaints,
    e) responding in pending proceedings,
    f) archiving.
  4. The legal basis for processing your data will be:
    a) necessity for the performance of the contract or to take steps at your request prior to entering into the contract,
    b) necessity for purposes arising from the legitimate interests pursued by the controller, e.g. responding to your letters and applications,
    c) necessity to comply with a legal obligation incumbent on the controller.
  5. Your data may be disclosed to the following recipients:
    a) hotels, airlines,
    b) processors in connection with activities commissioned by PoloniaMedica and carried out on behalf of PoloniaMedica (tour leaders, local carriers),
    c) entities authorised under applicable law,
    d) inspection and supervisory authorities,
    e) enforcement authorities.
  6. Your data will be transferred to third countries for the purpose of performing the contract, if the concluded contract concerns such a country.
  7. The retention period for your personal data is 5 years from the end of the financial year in which the contract was performed.
  8. In addition, we inform you that you have the right to:
    a) request access to your personal data,
    b) rectification of data,
    c) completion of incomplete personal data, including by providing an additional statement,
    d) erasure of data or restriction of processing,
    e) object to further processing of your personal data,
    f) data portability.
  9. You have the right to withdraw your consent at any time. Withdrawal of consent does not affect the lawfulness of processing carried out on the basis of consent before its withdrawal.
  10. If you have any doubts as to the correctness of the processing of your personal data by PoloniaMedica , you have the right to lodge a complaint with the supervisory authority.
  11. PoloniaMedica does not use systems for automated decision-making.
  12. Any correspondence regarding the processing of your personal data should be sent to the Controller's address with the note "Personal data" or to the e-mail address contact@poloniamedica.com.
