Information clause of the contact form

  1. The controller of personal data related to completing the contact form is “CZĘSTOCHOWA” Cargo & Travel Agency Jan Korsak, with its registered office in Częstochowa 42-200, ul. Św. Rocha 31, operating under the brand name “PoloniaMedica”, hereinafter referred to as “PoloniaMedica”.
  2. In matters of data protection, you may contact the Controller at the e-mail address contact@poloniamedica.com, or by post: PoloniaMedica, ul. Św. Rocha 31, 42-200 Częstochowa.
  3. Your personal data obtained in connection with completing the contact form will be processed in order to contact you.
  4. The legal basis for processing your data will be the necessity for the performance of a contract or to take steps at your request prior to entering into a contract, as well as the necessity for purposes arising from the legitimate interests pursued by the controller, e.g. responding to your letters and applications (Art. 6(1)(f) GDPR).
  5. Your data will not be disclosed to other entities and will not be transferred to third countries.
  6. The retention period for your personal data - for one-off contact (providing information), no longer than 180 days from the last contact.
  7. You have the right to withdraw your consent at any time.
  8. Withdrawal of consent does not affect the lawfulness of processing carried out on the basis of consent before its withdrawal.
  9. In addition, we inform you that you have the right to:
    a) request access to your personal data,
    b) rectification of data,
    c) completion of incomplete personal data, including by providing an additional statement,
    d) erasure of data or restriction of processing,
    e) object to further processing of your personal data,
    f) data portability,
  10. If you have any doubts as to the correctness of the processing of your personal data by PoloniaMedica, you have the right to lodge a complaint with the supervisory authority. PoloniaMedica does not use systems for automated decision-making.
